Patients must be at least 18-years-old to receive medications. Medications are prescribed four weeks at a time, with one course of treatment not exceeding 12 weeks. Patients are eligible for this treatment once every 6 months. This coverage was added in 2008.

Medications are provided at no cost to the member, up to a $500 lifetime maximum. Phone counseling and coordinated care is provided through the Healthful Living Tobacco Free Program. Enrollment for this program is open January through March. This counseling is not required to receive medications.

Private Insurance Coverage
This state does not require private health insurance plans to cover cessation treatments. Cessation coverage in private health insurance plans varies by employer and/or plan. Smokers with this type of health insurance should contact their insurance plan for information on cessation benefits.
