Individual Counseling
Co-payments of $3 are required for all medications (except mail order prescriptions). All medications are limited to a 3-month duraction except for varenicline (however, members may only try varenicline once per lifetime). Members are covered for three individual counseling appointments per year. Prior authorization is required for the NRT nasal spray and inhaler, and members must try other medications first before choosing these treatments.

Anthem Blue Cross, Blue Shield will pay for cessation medications up to $200 per year and $400 per lifetime. The plan covers 2 follow-up visits with a member's physician for cessation counseling. It also covers enrollment in 2 group counseling classes per lifetime (provided through hospitals or physician's offices). Co-payments are also required.

Private Insurance Coverage
This state does not require private health insurance plans to cover cessation treatments. Cessation coverage in private health insurance plans varies by employer and/or plan. Smokers with this type of health insurance should contact their insurance plan for information on cessation benefits.
