Course of treatment for NRT patch is limited to 42 days. Course of treatment for other medications is limited to 84 days. One quit attempt per year is covered. Prior authorization is required if member wants to exceed duration limits. Co-payments of $1-3 are required.

Coverage for prescription cessation medications is only available to members of the Option IV plan. Members are responsible for 20% of the costs after deductible.

Private Insurance Coverage
This state does not require private health insurance plans to cover cessation treatments. Cessation coverage in private health insurance plans varies by employer and/or plan. Smokers with this type of health insurance should contact their insurance plan for information on cessation benefits.
